Clinical background

Inflammatory polyneuropathies can be severe and progress rapidly. Screening for antibodies associated with inflammatory neuropathy should be performed at an early stage of the disease to improve diagnostics and not delay initiation of proper treatment. Serum samples should always be drawn before treatment, which can compromise the serological test results.

Antibodies against gangliosides (GM1-, GT1a-, GQ1b-, GD1a-, GD1b-, GT1b- IgG and IgM) and MAG IgM are associated with motoric and sensomotoric inflammatory neuropathies. Antibodies against multiple gangliosides can often be detected simultaneously due to cross-reactivity as result of their high molecular similarity.

High levels of MAG IgM have been reported in demyelinating neuropathies. In patients with monoclonal IgM gammopathies and neuropathy, antibodies against MAG can be detected in about 50% of the cases.

Autoimmune nodopathies can clinically mimic acute demyelinating polyneuropathy and analysis of antibodies associated with this condition should be performed when suspecting inflammatory neuropathy. This includes IgG antibodies against Neurofascin 155, Neurofascin 186, Contactin1, and CASPR1.

Neurofilament light protein (NfL) is a protein in axonal cytoskeleton and in both the central and peripheral nervous system (CNS and PNS). The concentration of NfL in blood reflects the degree of axonal damage. The half-life of NfL is long and normalization of levels occurs about 6 months after the injury.
