Clinical background
Primary membranous nephropathy (PMN) is an antibody-mediated inflammatory disease of the glomeruli that leads to progressive loss of kidney function.
The inflammation is driven by the production of autoantibodies against the transmembrane protein phospholipase A2 receptor (PLA2R) or Thrombospondin Type 1 Domain Containing 7A (THSD7A), both of which are expressed on the surface of the podocytes in the glomeruli.
Antibodies against PLA2R and THSD7A are strong markers for primary membranous nephropathy (PMN), that forms complement-activating immune complexes deposited on basement membranes in glomeruli with tissue damage and proteinuria as a result. Co-occurrence of antibodies against PLA2R and THSD7A is very rare.
