Clinical background
Primary membranous nephropathy (PMN) is caused by autoantibodies against renal glomeruli. Antibodies against PLA2R are the most common, but antibodies against THSD7A, NELL1, and CNTN1 occur in 2–15% of cases.
NELL1 (Neural Epidermal Growth Factor-Like 1) antibodies are detected in 5–10% of patients with PMN. NELL1 antibodies may occur secondary to other autoimmune diseases and following treatment with thiol-containing drugs. Malignancy has been reported in 15–33% of patients with NELL1 antibodies.
The antigens CNTN1 (Contactin-1) and CASPR1 (Contactin-associated protein 1) are expressed in both renal glomerular podocytes and peripheral nerves. Antibodies against CNTN1/CASPR1 are associated with a distinct autoimmune syndrome characterized by the co-occurrence of PMN and chronic inflammatory demyelinating polyneuropathy (CIDP).
