Clinical background

Alzheimer's disease (AD) is characterized by amyloid-beta-(amyloid-β) plaques in the brain with aggregation of phosphorylated tau (p-tau) fibrils in neurons. These changes cause tissue damage and neuron loss. The pathophysiological mechanisms that lead to the neuronal damage are initiated many years before cognitive impairment. The diagnosis is based on cognitive, neurological, and physical examination as well as analysis of blood samples, biomarkers, and genotype.

Total Tau reflects axonal disintegration. Tau constitutes cortical, unmyelinated axons where they bind to microtubules with a stabilizing effect. Total Tau can be used as a biomarker in Alzheimer's disease; increased levels can also be seen in other degenerative diseases of the brain and stroke.

Phospho-Tau (p-Tau) reflects the hyperphosphorylation of Tau and the formation of neurofibrils. P-tau has a reduced ability to bind microtubules, which leads to impaired axonal stability. Elevated levels of Phospho-Tau are typical in Alzheimer's disease but rarely in other conditions.

Beta-amyloid ratio 42/40 In Alzheimer's disease, Beta-amyloid 42 accumulates in plaques, which leads to a lower ratio of Beta-amyloid 42/40 and can be detected early in the course of the disease.
