IndicationSuspicion of celiac disease (CD) or dermatitis herpetiformis.MethodIndirect immunofluorescence with sections of monkey oesophagus as substrate.AnswerThe result is given as negative, weak, medium or strong positive after evaluation of the fluorescence intensity.InterpretationAntibodies against endomysium are investigated in suspicion of celiac disease or dermatitis herpetiformis, where about 90% of the patients with celiac disease and 80-90% of dermatitis herpetiformis patients are positive. The antibodies disappear with gluten-free diet. Anti-endomysiums most important antigen in the tissue is transglutaminase 2. The test is negative in healthy subjects.Anti-endomysial antibodies are a strong marker for celiac disease. In cases of IgA-deficiency the result can be falsely negative and analysis of antibodies of IgG-class against endomysial tissue or transglutaminase 2 is then recommended.

Antikropparna försvinner vid glutenfri diet. Anti-endomysiums viktigaste antigen i vävnaden är transglutaminas 2. Testen är negativ hos friska personer.Antikroppar mot endomysium talar starkt för celiaki. Vid IgA-brist kan, av naturliga skäl, inte IgA-antikroppar påvisas och denna analys kan då bli falskt negativ. Analys av IgG-antikroppar mot endomysium eller transglutaminas 2 rekommenderas då i stället.
