Primary Membranous Nephropathy (PMN)
IndicationSuspicion of primary membranous nephropathy (PMN, previously idiopathic membranous nephropathy (IMN).
Clinical backgroundPrimary membranous nephropathy (PMN) is a chronic inflammatory disease of glomeruli that leads to progressive loss of kidney function. In addition, it is the most common reason for nephrotic syndrome. The root cause of the inflammation is due to the production of autoantibodies against the transmembrane protein phospholipase A2 receptor (PLA2R) and Thrombospondin Type 1 Domain Containing A7 (THSD7A), both expressed on the surface of the podocytes in glomeruli. Antibodies to PLA2R occur in serum in 75% of patients with PMN, while the presence of THSD7A is significantly lower (2.5-14% in different cohorts).
IgG class antibodies against PLA2R have high specificity for PMN. Very few healthy people and patients with other types of membranous nephropathy have antibodies against PLA2R. Antibodies of IgG class against THSD7A occur less frequently in PMN and, rarely, can be detected concurrently with antibodies against PLA2R.

KlinikPrimär membranös nefropati (PMN) är en kronisk inflammatorisk sjukdom i glomeruli som leder till progressiv förlust av njurarnas funktion. Dessutom är det den vanligaste anledningen till nefrotiskt syndrom. Grundorsaken till inflammationen beror på produktion av autoantikroppar mot det transmembrana proteinet phospholipase A2 receptor (PLA2R) och Thrombospondin Type 1 Domain Containing 7A (THSD7A), som båda uttrycks på podocyternas yta i glomeruli. Antikroppar mot PLA2R förekommer i serum hos 75% av patienter med PMN, medan förekomsten av THSD7A är betydligt lägre (2.5-14% i olika kohorter).
Antikroppar av IgG klass mot PLA2R har hög specificitet för PMN. Mycket få friska personer och patienter med andra typer av membranös nefropati har antikroppar mot PLA2R. Antikroppar av IgG klass mot THSD7A förekommer mindre ofta vid PMN och kan i sällsynta fall påvisas samtidigt med antikroppar mot PLA2R.
